Privacy policy

Template text. Have it reviewed for your practice and state before you publish.

Dental Care PC ("we", "us") runs this website for our dental practice. This policy explains what personal information the website collects, why, and the choices you have. It covers the website only; your care at the practice is covered by our Notice of Privacy Practices, which we give you at your first visit.

What we collect

Appointment requests (the booking form). Your name, email address and, if you give it, a phone number; whether you are a new or existing patient; the kind of visit you are asking about, chosen from a list; a preferred day and part of the day; any short note you write; and your agreement that we may contact you.

Emergency call-back requests. Your name and phone number, an email address if you give one, and a reason chosen from a short list (for example a broken tooth).

That is all. We do not ask for your medical or dental history, medications, insurance member numbers, date of birth or payment details online, and we ask you not to type them into the notes. We ask for those at your visit.

The form also has a hidden field that only spam bots fill in; we do not keep it.

Our hosting provider records basic technical data (IP address, browser type, pages requested) to keep the site secure and working.

Health information and your records

Your clinical records (X-rays, charts, treatment notes, insurance details) never live on this website. They are kept in the practice's own records system, protected as the Health Insurance Portability and Accountability Act (HIPAA) and state law require.

A website request is not a clinical record. We use it only to call or email you back and find a time. If it becomes an appointment, we copy what we need into the practice system and then delete the website request.

How we use it

  • To answer your request and offer you a time.
  • To call you back the same day about an emergency request.
  • To contact you about that request, for example to move the time.
  • To protect the site from spam and abuse.
  • If you accept analytics, to understand which pages are useful, in aggregate.

Cookies and analytics

The site works without optional cookies. Nothing optional loads until you choose: if analytics is switched on for this site and you click Accept in the cookie banner, we load Cloudflare Web Analytics, which counts page views without building a profile of you. If you click Decline, nothing optional loads.

Your choice is stored in your own browser. You can change it any time with the Cookie settings link at the foot of every page.

Who we share it with

We do not sell your information or use it for advertising. We share it only with services that help us run the site:

  • Website hosting and the database that holds requests until we answer them (for example Vercel).
  • Email delivery of requests to our front desk (Resend).
  • Spam protection on the booking form (Cloudflare Turnstile), when it is switched on.
  • Page-view analytics (Cloudflare Web Analytics), only when it is switched on and you accept it.

The Get directions link takes you to Google Maps; Google's own privacy policy applies there.

How long we keep it

Website requests are deleted within 90 days of the day you asked for, or sooner once they are copied into the practice system.

Your rights

You can ask to see, correct or delete the information this website holds about you, or object to how we use it. Email hello@example.com and we will reply within 30 days.

California privacy rights (CCPA/CPRA)

If you live in California, you have the right to know what personal information we collect and how we use it, to ask for a copy, to have it corrected or deleted, and to opt out of its sale or sharing. We do not sell your personal information or share it for cross-context behavioral advertising, and we do not knowingly collect information online from anyone under 16; a parent or guardian books for a child.

To use these rights, email hello@example.com. We will confirm your request and reply within 45 days. You can use an authorized agent, and we will not treat you differently for using any of these rights.

Global Privacy Control

We honor the Global Privacy Control (GPC) signal. If your browser sends it, we treat it as a request to opt out of sale and sharing, and we keep optional analytics off for that browser without asking.

Changes and contact

We will post any changes on this page with a new date. Questions? Contact Dental Care PC at hello@example.com, 120 Sample Street, Suite 300, Sample City, CA 94000.
